Death Registration (CRDA)

General Information

Any consular services require a prior appointment. To make an appointment please call the Foreign Interests Section.

Please make sure to be on time for your appointment; late arrivals or no shows need to make a new appointment. If you have to cancel your appointment please inform this section at least 24 hours in advance. 

The Consular Report of Death Abroad (CRDA) is a document necessary to settle legal and estate matters in the United States.

The U.S. authorities will issue up to 20 copies of the Consular Report of Death Abroad.

The following documents are required in order to register the death of a U.S. citizen in Iran:

  • Original U.S. passport of the deceased
  • Original U.S. birth certificate or U.S. certificate of naturalization of the deceased
  • Original U.S. social security card of the applicant, if available or its number
  • Original Iranian death certificate in which the cause of death is clearly mentioned
  • Official English translation of the Iranian death certificate, certified by the Iranian Ministry of Foreign Affairs
  • Original of the cancelled Iranian birth certificate of the deceased in which the date of the death is entered
  • Official English translation of the cancelled Iranian birth certificate of the deceased, certified by the Iranian Ministry of Foreign Affairs
  • Original Iranian passport of the deceased
  • Last known addresses of the deceased in the U.S. and in Iran
  • All personal information of the applicant as required by the Report of Death Questionnaire (PDF, 2 Pages, 404.5 kB, English)
  • Power of Attorney, Will or Trust specifying the disposition of the estate or if unavailable notarized form DS-5511 "Affidavit for the surviving spouse or next of kin"
