The project aims to improve the offer, coordination and outreach of community health and social services to the most vulnerable people by capacity building local authorities (planning, management, coordination) and service providers (GP, social services, community nursing, auxiliary services).

Attività

Local authorities are strengthened to conceptualise, implement and monitor local integrated care projects. This includes capacity building, Coaching and monitoring of local stakeholders and authorities. Existing services will be assessed and additional services developed where needed targeting most vulnerable populations, The implementation is followed through baseline and follow up surveys to assess effectiveness of the intervention. Based on its findings, models including activity packages and lessons learnt will be developed and communicated at the end of the project phase.

Risultati

Risultati raggiunti:  

  • In cooperation with Swiss Tropical and Public Health Institute (TPH), 7 pilot community medico-social service projects have been implemented in the regions of Tulcea, Botosani and Salaj and provide integrated services to people living in rural communities with the focus to improve the access, quality and comprehensiveness of services for people in need 
  • 7 integrated community medico-social services where set up. Training programs were conducted for more than 60 community nurses in these three districts. Capacity building for project local authority teams was conduct at the beginning of the project
  • On average 10 coaching and monitoring visits where conducted per project site and more than 20 joint workshops for community actors
  • Vulnerable groups have appropriate access to and make use of basic health and social services at community level. More than 700 people in need from different vulnerable groups (children, elderly, pregnant women, people with social assistance, chronically ill, families with special needs) profited from services of the 7 community centers during the lifetime of the project 
  • Community integrated service models have been developed and guidelines for the development of such models for local authorities are developed. The capitalization report with lessons learnt was developed have been established. 
  • A framework for the establishment of community integrated care centres for a broader access to the Romanian population – especially for vulnerable groups – has been created and approved. Two national EU funded projects are to be launched for local authorities to disseminate this approach (with 2 components: infrastructure and community nurses training).
  • Normatives for community nursing activities were developed and have been approved by national authorities
  • Dissemination of project experience disseminated through international conference of integrated care in San Sebastian 2019
